UnitedHealthcare® Group Medicare Advantage HMO (PMA).

PEBC PPO Plan
Available only to dependents not entitled to or eligible for Medicare as long as retiree is enrolled in either the UnitedHealthcare Group Medicare Advantage (PPO) or the UnitedHealthcare Group Medicare Advantage (HMO). We refer to the combined plan as MPD (retiree enrolled in Medicare Advantage PPO with spouse enrolled in PPO) or PMD (retiree enrolled in Medicare Advantage HMO with spouse enrolled in PPO). If you enroll your spouse in the PPO Plan, a Spouse Medical Plan Surcharge Affidavit is required.
